As the Clinical Discharge Lead for Lancashire Teaching Hospitals you will work across the Central Lancashire health and social care system with acute, community and social care partners to ensure that patient are cared for in the most appropriate environment, that a full discharge to asses model is implemented in Central Lancashire and patients time in hospital is limited to only that which requires acute hospital intervention in order that they receive the best patient experience possible, improving outcomes.
You will be responsible for operationally managing our inpatient discharge teams and our exciting new development, hospital homecare, whilst driving improvement across the Central Lancashire footprint. This will include working closely with system partners.
You will have expert working knowledge and extensive clinical expertise as a Nurse or AHP in discharge processes and regulations across community and acute settings and have the ability to design with partners and patients, pathways and systems in and outside of hospital to ensure the citizens of Central Lancashire receive a high quality experience of discharge, spending the shortest time possible in the hospital setting.
You will be an experienced leader of people and pathways, having gained considerable exposure to senior NHS settings, regulatory exposure and be working at an 8b level or above currently.
You will have the skills to operationally manage a number of teams and services whilst delivering change at pace using quality improvement mythology.

As a registered body with the DBS, Lancashire Teaching Hospitals NHS Foundation Trust will conduct the administration of this application on your behalf and the cost will be recovered from your salary, either as a one-off payment or over 3 months.
