An exciting opportunity has arisen for an experienced vibrant registered nurse who is seeking to make the next step in their palliative career to join our team based at St Mary’s Hospital NHS Trust.
As the new Mountbatten Palliative Care Discharge Coordinator, you will actively support discharge planning and facilitation, you will help to prevent inappropriate admissions into St Mary’s Hospital and ensure that care at home is a viable option for people approaching the end of life.
You will be working in partnership with Social Care, Primary Care, NHS Providers and NHS Continuing Healthcare in facilitating integrated patient focused service provision. Whilst an employee of Mountbatten, you will be a core member of the Integrated Palliative and End of Life Care Team within St Mary’s Hospital.
Experience of collaborating with other health and social care providers in primary, secondary, independent and voluntary care is essential. You will ensure communication links are in place to deliver a continuous, well-coordinated and seamless discharge process as well as tracking those patients appropriately admitted into the hospital.
An effective advocate for the patient, you will understand the needs of those receiving palliative/end of life care as well as governance issues, risk management and audit procedures.
