HART Team Lead

The Home Assessment Reablement Team (HART) is a community based service that provides reablement support to service users from a wide range of hospital and community based settings within their own home across Oxfordshire. The service also supports service users in the community to prevent hospital admission. Close links have been developed with our community colleagues to ensure a smooth handover to any appropriate service and to prepare patients and clients for their future pathway.

The service has three bases, John Radcliffe Hospital (covers Oxford city), Horton General Hospital (covers North and West Oxfordshire) and Abingdon Health and Wellbeing Centre (covers South Oxfordshire). HART is looking to recruit to the West Team Lead post, to manage the services the West Oxfordshire team. You will be primarily based from the Horton General Hospital as well as in the community across West Oxfordshire, however you will be required to have the flexibility to work from any of the bases and in the community across Oxfordshire, depending on the needs of the service.

The service facilitates the discharge of service users from acute and community hospitals who require some additional support or who are awaiting long term care packages/further rehabilitation at home.

The Team Lead is responsible for ensuring effective, safe and responsive service delivery to the service users and staff in the locality team through strong leadership. The post holder will be required to work jointly and cohesively with other HART Team Leads and HART Senior Management Team, to ensure consistency and quality across the county.

It is essential that you will have previous line management experience and are able to demonstrate effective communication, excellent organisational skills, be flexible and adaptive, used to working in and dealing with highly stressful situations and maintaining a calm and professional manner.

The role will be both office and community focused and full training and support will be provided.

The service operates 365 days a year from 7am–10pm. You will work on a rota basis including weekends and bank holidays.

It is essential that you hold a full UK driving licence and have a car available for work at all times, due to the community based side of this role.
