The Memory Service base is on the Longley Centre site in the grounds of the Northern General Hospital and offers assessment and post diagnostic support through the provision of the Case Management team or the delivery of psychosocial/education interventions to both patients with a diagnosis of dementia and their carers either on a group or individual basis.
Working as part of the multi-disciplinary team you will provide clinical support to junior members and managerial support in the absence of the Service Clinical Manager. You will provide comprehensive assessments, pre and post diagnostic counselling, and participate in the psychosocial/educational interventions and, work closely with the local authority, and community agencies. You will also be involved in developing new initiatives in this ever changing service. Your role will also consist of participating with colleagues in the promotion of an active learning environment to maintain and develop high standards of professional and clinical practice, contribute in the development of others through supervision and appraisal and partake in audit service evaluation and national projects. You will have active daily responsibility to participate and coordinate the delivery of the Case Management Team.

You will need to be an RNMH with an interest in organic illness together with evidence of higher practice skills in dementia care and evidence of continuous professional development. You should have experience of working in a multi-disciplinary/multi-agency setting with experience of practice supervision and risk management.
It is essential that you hold a full driving license and have access to a car.
