AHP or Nursing Team Lead for Urgent Community Response Join our teams as we launch a brand new service to provide Urgent Community Response for the population of Mid Hampshire, covering the Andover and Winchester areas. The Team Lead role in Urgent Community Response is a really exciting and dynamic way to work with people to support them to receive high quality and advanced level care in their own home. Urgent Community Response delivers holistic and specialist assessment and treatment to people who are at risk of hospital admission. The aim of the team is to enable people to stay at home, recover, and re-engage with the important aspects of their life and prevent admission to acute sectors. As the team lead you will operationally lead Urgent Community Response and work clinically to support the development of the service alongside Advanced Clinical Practitioners and Consultant Practitioners. The role allows you to work autonomously and implement a breadth of interventions, while having the support of a wider multidisciplinary team, including nurses, AHP’s and Consultant practitioners. The role is rewarding as it allows you the time and scope to explore what’s important for each person and work with them to improve their health and well-being.
