An exciting opportunity has arisen within our Community and District Rehabilitation Team, working within Carlisle, Longtown, Brampton and Penrith we are looking for a motivated and enthusiastic Home Care Practitioner to work as part of our dedicated team.
The intention of the service is to support people for a short time in the community following a period of ill-health, to encourage return to independence which is part of the Government’s Closer to Home agenda. Also to support some Palliative patients in the end of life stage.
We play a key role in preventing hospital admissions and facilitating safe, supported, prompt discharges from the hospital, working closely alongside the, Home First and the Adult Social Care Re-ablement Service.
Effective communication both written and verbal is essential.
The role will require you to work unsupervised as a lone worker, working within your own limitations and competencies.
The role will also require a flexible outlook that will meet the requirements of the locality.

The Trust is committed to safeguarding adults and children therefore if the post is subject to the Rehabilitation of Offenders Act (ROA) 1974 (Exceptions) Order 1975 (as amended) the successful candidate(s) will be subject to a criminal records check with the Disclosure and Barring Service (DBS). The cost of the DBS must be met by the successful candidate(s) through salary deduction. DBS charges are as follows if applicable to the post:
Standard Check: £26 + Administration cost of £4.20 = deducted at £10.20 from 1st month salary and £10 deducted from salary over following 2 months.
Enhanced Check: £44 + administration cost of £4.20 = £15.20 deducted from 1st month salary and £11 deducted from salary over following 3 months.
