Berkshire West Clinical Commissioning Group

NHS Berkshire West CCG is recruiting a Nurse Assessor to carry out Children and Young Person's Continuing Care assessments and case management. The role is based in Reading, Berkshire but involves travel over Reading, Wokingham, Newbury and surrounding areas.

This successful candidate must be registered as a Paediatric Nurse.

To be an integral part of Berkshire West CCG's Continuing Healthcare and Funded Nursing Care Team. The post holder will assist in the implementation of the Children and Young Person's Continuing Care National Framework.

You should have excellent communication skills with the ability to produce clear and concise written reports. You should also have excellent interpersonal skills to support you in managing complex discussions with a range of agencies and individuals including care providers and relatives. Previous experience of working in Children and Young Person's Continuing Care although not essential, would be an advantage.

Experience of undertaking care needs assessments, case and risk management will be essential for this role.

Post holders will be expected to travel between sites and on occasion may be required to work from a different office base so own transport is essential.
