Complex Care Coordinator HFH Healthcare Limited is a caring independent Complex Healthcare organisation which deliversthe highest quality care to extremely vulnerable adults & children in the comfort and security of their own homes, throughout Central and Greater London. Working in close partnership with the NHS continuing healthcare teams, our enhanced level of specialist nurse directed clinical support enables our clients to remain safe at home even when the level of health care need required is highly complex. Role profile: This is an exciting time to join our organisation following a period of internal review and improvements. You will be responsible for supporting and managing a care portfolio of clients including regular touchpoints and client visits so communication and being confident in a client facing role is essential. You will work closely with your clients and the supporting carers to ensure a stable team of carers is always rostered in and well in advance and ensure that all parties are in the know at all times. As part of the ongoing support for your carers within your care portfolio you will be accountable for conducting ongoing carer supervisions and annual appraisals for all carers within your portfolio. You will be adept at problem solving and identifying issues arising within your care portfolio to ensure that continuity of care for your clients is always maintained. All tasks being delivered will need to be with a mindset of delivering the highest quality care and customer service excellence.
