We are looking for a Mental Health Nurse/Practitioner to join one of our multi-disciplinary teams, within HMP Leyhill/Ashfield.
The post holder will be working within primary and secondary care as part of an integrated mental health and substance misuse services. They will provide the assessment and planning function for referrals to the service, support the Independent Nurse Prescriber and, working with the support of Consultant Psychiatrist, management, wellbeing support workers and peer supporters, they will develop the care model and form an integral part of the care program approach.
The post holder will be a central role within the team working to enhance and maximise the potential and recovery of people within the service. The post holder will also support other registered and unregistered staff with tasks to ensure the smooth running of the care that we provide.

You will have excellent communication skills and be committed to the provision of a recovery-focused model of care, within an integrated physical and mental health service.
You will be based with our multi-disciplinary team within the prison and will work closely with a variety of other professionals, including Psychiatrists, Mental health nurses, Neurodevelopmental Practitioners, Wellbeing support workers, Intensive Recovery Co-coordinators and peer mentors. The role will involve collaborative work with all disciplines within the prison including offender management (probation), psychology, programs and prison officers. There is an excellent multidisciplinary approach towards patient care and an exciting opportunity to work closely with different professions.
HMP Leyhill is based in a beautiful rural setting in Gloucestershire and is a short journey from the surrounding cities including Gloucester and Bristol. There is free parking on site, a café and access to a gym.
The successful candidate will be required to obtain prison security clearance in addition to DBS and reference checks.
