Huntingdon PCN – Social Prescriber Link Worker Position
West Cambs Federation exists to support the delivery of Primary Care across Huntingdonshire and Fenland.
After implementing the findings of a Governance Review, we recently restructured our Board and have been working on our future ambitions. In March 2020, we were rated as GOOD by the CQC.

West Cambs Federation and Huntingdon PCN, work in collaboration and we seek a full time Social Prescriber to join our expanding team. The candidate must be able to demonstrate that they meet the all of the criteria within the person specification. They should also be able to demonstrate the ability to cope in a busy and demanding environment.
Huntingdon PCN covers 4 GP Practices (Acorn, Priory Fields, Charles Hicks and Papworth) and serves a patient population of 45,000. There is a full team of GP partners, supported by Clinical Pharmacists, Pharmacists, Advance Nurse Practitioners, Nurses and an HCA plus a large team of admin support.
Salary £21,000 - £24,000 depending on experience.
