This is a newly created post to support new acute and rehabilitation inpatient facilities and to ensure the seamless transfer of patient’s pharmaceutical care needs to and from the hospital setting. You will become a fully integrated member of ward multidisciplinary teams in order to deliver a high quality medicines optimisation and management service in line with departmental and Trust objectives.
The post-holder will deliver a medicines optimisation service focused on ensuring patient’s get the best from their medicines and that other healthcare professionals are well informed and skilled to be able to manage the use of medicines correctly, safely and cost effectively.
You will be part of a wider, expanding Pharmacy Team that is highly regarded within the organisation and is looking forward to exciting times ahead. EPMA implementation has gone live, and will become embedded in practice across the Trust in the coming months.
